If you wish to utilize your health insurance service or employee benefit plan which may provide coverage either in full or in part, I will provide a master billing statement which you can submit to your health insurance or benefit plan provider for reimbursement. 

Please check your coverage carefully by asking the following questions:

• Do I have mental health benefits as part of my plan?
• What is my deductible and has it been met - is there a seperate deductible for mental health benefits?
• How many sessions per calendar year does my plan cover?
• What is the total coverage amount? Amount per therapy session?
• Is approval required from my primary care physician, before I begin therapy?
• How much does my plan cover for an out-of-network provider?Additionally, some health insurance and employee benefit plan providers may include therapy sessions as part of your permanant medical record. Be sure to check with your provider to verify if this is or is not the case.

Cancellation Policy

24 hours notice is required for the cancellation of any appointment. Failure to do so will result in your being billed in full for the missed / uncancelled session(s).
